Botox can be a nice option for softening certain wrinkles, but I’ve learned that more isn’t always better, especially as our faces change with age.

I’ve tried Botox myself, and while I’ve been happy with some results, I’ve also had experiences that made me more careful about where and how much I use.

So, if you’re thinking about trying Botox after 40, or wondering whether it’s still worth getting, I’ll walk you through 10 possible benefits, along with a few mistakes and downsides worth knowing about.

I’ll also share some of my own experiences and insights from cosmetic professionals, so you can get a better idea of what Botox might help, what it won’t fix, and whether it makes sense for you.

10 Benefits of Botox: and 4 mistakes that could age your face.

If you’re weighing Botox alongside skincare, devices, or at-home treatments, it helps to step back and look at the bigger picture first. My skin support overview explains how I think through options like injectables — what’s worth considering now, what can wait, and what often gets overhyped.

How Botox works to smooth facial wrinkles

Botox can help soften expression lines, especially across the forehead, around the eyes, and between the eyebrows. It may also help keep some of those lines from becoming more noticeable over time.

But here’s the catch: Too much Botox, or injections in the wrong spots, can sometimes leave your face looking stiff or make certain areas look heavier. That’s why the amount used and where it’s placed really matter, especially as our faces change with age.

Today there are several forms of purified botulinum toxins with different strengths. These differences are useful in our practice since all muscles have slight variations.”
— Dr. Patricia Wexler, Dermatologist

Botox pros and cons: What’s worth it (and what’s not)

The upside: Botox can soften expression lines without surgery, and there’s usually little downtime. Results typically last about three to four months.

The downside: The cost can add up because you need repeat treatments to maintain the results. And depending on where it’s injected, Botox can sometimes cause drooping, unevenness, or changes in your facial expressions.

For me, it’s not just about smoothing a wrinkle. It’s about whether the result will look natural and make sense for my face.

10 Benefits of Botox for facial aging

Botox can help with several age-related changes, but not every treatment makes sense for every face.

Here are 10 possible benefits, including a few you might not know about. I’ll also point out where results may be subtle, what Botox can’t do, and a few things worth considering before spending your money.

1. Smooths forehead wrinkles

Botox relaxes the frontalis muscle to soften those horizontal lines across your forehead. It also slows the deepening of creases over time — especially helpful if you raise your brows a lot without realizing it.

2. Softens crow’s feet

Tiny Botox injections around the outer corners of the eyes can soften those little lines that form when you squint or smile. When done carefully, they can smooth the area while still allowing some natural movement.

I used to get Botox for my crow’s feet, but after consistently doing eye massage and facial exercises, they’re not so bad. It’s one of the areas where natural methods made a little difference for me.

3. Relaxes frown lines (the “11s”)

A little Botox between the brows can soften that tired or tense look. It works by relaxing the muscles that pull inward, so your face can look more relaxed without losing its natural expression.

I honestly love this one. After using it consistently, I only need a touch-up once or twice a year now. For me, that’s been enough to soften those lines without overdoing it.

4. Softens under-eye “jelly roll” puffiness

Ever notice a little roll of skin just under your lower lashes when you smile? That’s sometimes called a “jelly roll.”

In some cases, a very small amount of Botox can soften that roll by relaxing the muscle underneath. But this is a delicate area, and Botox can sometimes make lower-eyelid looseness or puffiness more noticeable, especially as our skin changes with age.

It’s also important to know that a jelly roll isn’t the same as ordinary under-eye bags. I explain the difference on my Botox for eye bags and puffiness page.

5. Minimizes Lip Wrinkles

A small amount of Botox around the mouth may help soften those little vertical lines above the lips, sometimes called “smoker’s lines,” even if you’ve never smoked.

But the mouth area is tricky. Too much Botox can affect how your lips move when you smile, speak, or drink through a straw. That’s why I’d be especially careful about this area and choose someone experienced with treating it.

6. Gently Lifts the Corners of the Mouth

A small amount of Botox in the muscles that pull down the corners of your mouth may help soften that downward turn some of us notice with age.

It’s a subtle change, not a full lower-face lift. And because these muscles are close to others that control your smile, careful placement really matters. Too much Botox or an injection in the wrong spot could leave your smile looking uneven.

7. Improves Skin Tone and Firmness

Here’s an interesting benefit you might not know about. A small study published in JAMA Facial Plastic Surgery found that Botox improved certain measurements of skin elasticity and flexibility in the treated areas.

That’s encouraging, but it doesn’t mean Botox can tighten loose, sagging skin or replace lost facial volume.

So while Botox may offer some skin benefits beyond smoothing expression lines, I wouldn’t count on it as a skin-tightening treatment. You can read the study here.

8. Creates a Fuller Look with a Lip Flip

A Botox lip flip can make your upper lip look a little more noticeable, especially if it tends to disappear when you smile.

It works by relaxing the muscles around the upper lip, allowing the lip to roll slightly outward. It doesn’t add volume like filler, so the change is usually subtle.

One downside worth knowing about: A lip flip can sometimes make drinking through a straw, sipping from a cup, or certain lip movements feel awkward until the Botox wears off.

If you’re curious about the difference it can make, take a look at these lip flip before-and-after photos.

9. Softens Neck Bands

Botox can help soften those vertical neck bands that become more noticeable with age. These are caused by the platysma muscle, which runs down the front of your neck.

Relaxing those bands may also help the jawline look a little more defined in some people. But here’s the important part: Botox won’t remove loose, hanging neck skin or get rid of a turkey neck caused by skin laxity.

If you’re not sure what’s causing the changes in your neck, I explain the differences on my turkey neck page.

10. Combines Well with Other Treatments

Botox can sometimes be combined with treatments like fillers, laser resurfacing, or microneedling, since each works on different concerns.

For example, Botox helps soften expression lines, while fillers can replace lost volume. But that doesn’t mean you need both.

Especially after 40, I think it’s worth considering what your face really needs before adding more treatments. Sometimes one carefully chosen treatment is enough.

If you’re curious, I’ve put together a guide to combining Botox with other treatments, including what to consider before trying more than one.

Note: Some Botox uses mentioned above, such as lip flips and certain injections around the mouth or lower eyelids, are off-label. That means they aren’t specifically FDA-approved for those purposes. If you’re considering one, ask your injector about the benefits, risks, and whether it makes sense for your face.

4 Botox Mistakes That Can Affect Your Results

Even when Botox smooths wrinkles nicely, the results aren’t always what we expect.

Here are four things that can go wrong, especially around the cheeks, eyes, and brows, and what to keep in mind if you want natural-looking results.

1. Cheek wrinkles, hollow cheeks, or a strange smile

Botox around the outer corners of the eyes can soften crow’s feet, but placement matters, especially as our cheeks naturally lose some fullness with age.

If the injections affect muscles involved in smiling, you might notice an uneven smile, unusual cheek wrinkles, or a change in how your cheeks look when you smile.

That’s why I’d be careful about treating this area too aggressively. Sometimes keeping a little natural movement looks better than trying to smooth every line.

Take a look at the photo below, courtesy of Dr. Sarah Hart. It’s a good reminder that treating one area of the face can sometimes affect how another area looks.

2. More under-eye wrinkles

Botox can smooth crow’s feet, but sometimes treating that area changes how the skin under your eyes looks when you smile.

You might notice more fine lines or bunching beneath the lower lashes, especially if the skin is already thin or crepey. I’ve shared more about those criss-cross under-eye lines and why they can become noticeable.

That’s something worth discussing with your injector beforehand. Smoothing one area doesn’t always mean the surrounding skin will look smoother, too.

3. Heavy or droopy brows

Botox can do a nice job smoothing forehead wrinkles, but too much can sometimes make your eyebrows feel heavy or look lower than usual.

This is especially worth considering if your brows already sit a little low or you tend to raise your forehead to keep your eyes looking more open.

That’s why I’d ask your injector how treating your forehead might affect your brow position before getting Botox in that area.

4. Wrinkles showing up in new places

When Botox relaxes one group of muscles, nearby muscles may start working differently. Sometimes this can make wrinkles in other areas more noticeable, even if the treated area looks smoother.

Too much Botox can also leave your face looking stiff or less expressive than you expected.

What to do instead:

Ask your injector about using smaller amounts of Botox, especially around the eyes. You can read more about this lighter approach in my article on Baby Botox vs. Regular Botox.

Cosmetic medical expert Dr. Sarah Hart also discusses using Micro Botox around the crow’s feet, sometimes along with filler to restore lost volume around the temples and upper cheeks.

The goal is a softer, more balanced look without overdoing the Botox.

Note: Results vary based on individual factors, provider expertise, and treatment protocols. Consult a licensed professional.

10 Benefits of Botox: FAQs

A: It’s generally a good idea to skip saunas, hot tubs, and very hot environments for the first 24 hours after Botox, mainly to help limit flushing, swelling, or bruising.

Some injectors recommend waiting 48 hours, especially if you tend to bruise easily.

You don’t need to avoid all normal activity, but it’s worth following your injector’s specific aftercare instructions, particularly when it comes to strenuous exercise.

A: It depends on which antibiotic you’re taking.

Most antibiotics aren’t known to interfere with Botox, but certain types, especially aminoglycosides such as gentamicin, tobramycin, and amikacin, may increase its muscle-relaxing effects.

This could raise the risk of unwanted muscle weakness or other side effects.

If you’re taking antibiotics, let your Botox injector know which medication you’re using before your appointment. They can help you decide whether it’s safe to go ahead or better to wait until you’ve finished treatment.

A: It’s generally a good idea to skip alcohol for about 24 hours after Botox, especially if you tend to bruise easily.

Alcohol can temporarily increase flushing and may contribute to bruising or swelling around the injection sites.

Having a drink won’t necessarily ruin your Botox results, but giving your skin a little time to settle is a sensible precaution. Always follow your injector’s aftercare advice.

A: No, Botox can’t reverse aging, but it can temporarily soften certain wrinkles and make your face look smoother.

It works best on expression lines, like forehead wrinkles, crow’s feet, and frown lines. It won’t replace lost facial volume or tighten loose, sagging skin.

Once Botox wears off, muscle movement gradually returns, and wrinkles may become noticeable again. Your face will continue to age naturally, whether you use Botox or not.

A: The three most common areas are forehead lines, crow’s feet (around the outer corners of the eyes), and frown lines (those little vertical “11s” between your eyebrows).

These are popular treatment areas because Botox works best on wrinkles caused by repeated facial movements, like squinting, frowning, and raising your eyebrows.

Of course, how much Botox you need, or whether you need it at all, depends on your facial muscles and the results you’re hoping for.

A: Botox may offer a few skin benefits beyond smoothing wrinkles.

A small study published in JAMA Facial Plastic Surgery found that Botox temporarily improved skin elasticity and flexibility in treated areas.

But that doesn’t mean Botox will improve your overall skin health, tighten sagging skin, or replace a good skincare routine.

I’d think of any skin-firming benefits as a possible bonus, not the main reason to get Botox.

A: Unfortunately, no. Once Botox has been injected, there’s no proven way to reverse it or make it wear off faster.

The good news is that the effects are temporary. Most Botox results last about three to four months, although some movement may return sooner.

If you’re unhappy with your results, talk to your injector before trying to fix anything yourself. Depending on the problem, they may be able to suggest ways to improve the appearance while you wait for the Botox to wear off.

And for next time, consider starting with a smaller amount. You can always discuss adding more later, but you can’t take it away once it’s injected.
